• Identify problems needing immediate attention.
• Validate diagnoses with patient/family and health care team.
• Document diagnoses to support outcome planning.
• Develop and update an individualized plan of care.
• Involve patient/family in discharge planning from admission.
• Provide patient education based on evidence-based practice.
• Document all assessments, interventions, and responses.
• Reassess nursing actions for effectiveness toward outcomes.
