Responsibilities

• Screen/evaluate patients with cognitive or ADL impairments and develop care plans.
• Use standardized assessments and evidence-based practices for interventions.
• Provide rehabilitative treatments per regulatory requirements.
• Perform ADL retraining, therapeutic activities, exercises and modalities.
• Assess progress and adjust treatments in medical records.
• Evaluate outcomes and refer as needed for further care.
• Counsel patients and families regarding physical disorders.
• Communicate progress and plans with supervisor and health team.
