Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to join an in-house Skilled Nursing Facility in Oakdale, NY. This Weekend PRN role focuses on evaluating residents, developing personalized treatment plans, and delivering high-quality therapy in alignment with CMS expectations and facility standards.

You will document thoroughly in NetHealth EMR, collaborate with an interdisciplinary team, and pursue ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and treatment plans to residents and interdisciplinary team members.
