Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to join our in-house team at the Oakdale, NY Skilled Nursing Facility. You will provide comprehensive physical therapy services, conduct resident screenings, and contribute to collaborative care planning with the interdisciplinary team.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary documentation in CMS and NetHealth EMR systems.
