Cornerstone Clinical Partners, a physician-led group serving skilled nursing and long-term care facilities in North Carolina and Virginia, is seeking a Primary Care NP/PA to deliver on-site care in long-term care settings.

This part-time weekend role offers meaningful patient relationships, collaborative care, and the autonomy to practice at the top of your license, with strong clinical leadership and supportive operations across a defined region.

Competenze

Active and unrestricted NP/PA/physician license.
North Carolina license in good standing.
Minimum 6 months of experience in long-term care or skilled nursing facility.
Strong communication, clinical, and critical thinking skills.
Passion for elder care and person-centered healthcare.
Prolonged standing/walking; occasional lifting up to 20 lbs; computer work.

Mansioni

Provide primary care services to residents in skilled nursing and long-term care facilities.
Perform physical exams and medical history assessments.
Diagnose and treat chronic and acute health conditions.
Order and interpret diagnostic tests and lab results.
Develop and adjust individualized care plans.
Prescribe medications and monitor treatment effectiveness.
Collaborate with facility staff and interdisciplinary teams.
Educate patients and families about treatment options and care goals.
Ensure a safe, respectful, and supportive environment for residents.
