Job summary

Community Health Center, Inc. is seeking an LPN to provide weekend clinical support for primary care teams, helping implement planned care and maintain patient flow.

You will perform pre-visit preparation, document vital signs and chief complaints, conduct CLIA-waived tests, assist with immunizations and medication administration under standing orders, and help coordinate tests and referrals. Strong teamwork, patient communication, and familiarity with electronic health records are required; CPR

Qualifications

• LPN Program Completion Certificate.
• Skills include EKG use, appointment scheduling, word processing and computer skills, and familiarity with medical testing procedures.
• Sound organizational skills and accuracy; strong communication and customer service.
• Team oriented with leadership abilities.
• Bilingual preferred; expert knowledge of electronic health records (EHR).

Responsibilities

• Apply evidence-based guidelines and perform pre-visit preparation for primary care patients.
• Collaborate with provider using the Planned Care Dashboard to ensure complete visits.
• Prepare patients for visits and document chief complaints and vital signs.
• Independently perform CLIA-waived tests, EKG, hearing and vision tests.
• Prioritize daily tasks, coordinate tests and referrals, and maintain patient flow.
• Maintain a clean, safe work environment and medical logs; stock exam and lab rooms.
• Open/close med room and lab on weekends with proper log entries.
• Adhere to infection control standards and maintain CPR certification.
• Provide excellent customer service and teamwork, supporting other staff.
• Participate in in-service trainings and assist with medication administration and immunizations under standing orders.
