Job description

BAYADA Home Health Care

is currently seeking an experienced Weekend Start-of-Care Physical Therapist (PT) for a part-time opportunity performing home health visits for adult and geriatric clients in Frederick County.

Home Health and OASIS experience required!

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans with manual exercises, gait training, balance, and other interventions.
5. Assess and revise care plans, collaborating with nursing staff as needed.
6. Educate clients and family members on rehabilitative care and activities.
7. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

1. Current Maryland Physical Therapist (PT) license.
2. At least one year of recent PT work experience.
3. Graduation from an American Physical Therapy Association-approved program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency in computer skills; experience with EMR preferred.
