Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity required.
• Immunization and non-vaccine injectable certification required depending on area of practice.
• Credentialing and privileging required based on area of practice.
• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy or FPGEC from NABP.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provides patient education to patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens based on biomedical literature.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergencies and supports medication safety and quality improvement activities.
• Collaborates with the healthcare team to manage formulary adherence and medication-use process.
• Prepares, dispenses, and oversees storage/handling of medications following regulations.
• Serves as a preceptor for pharmacy students, residents, and learners.
