Job summary

Hope Hospice in Pleasanton, CA is seeking an After-Hours Licensed Vocational Nurse (LVN) to provide skilled patient care under the supervision of a Registered Nurse, following the written plan of care.

This part-time position requires completion of an LVN program and an active California LVN license; home health/hospice experience is preferred. Schedule is Saturdays and Sundays, 8:30am–5:00pm, with a pay range of $37.65–$47.00 per hour.

Qualifications

• Completion of a Licensed Vocational Nursing Education Program from an accredited school.
• Minimum of one year LVN experience, preferably in acute care or hospice settings.
• Current California LVN license and eligibility to practice.

Responsibilities

• Assist a registered nurse in providing direct patient care.
• Initiate communication with physicians and managers to facilitate optimal care.
• Instruct caregivers in care provision based on the Plan of Care under RN direction.
• Attend patient conferences and participate in care discussions.

We are seeking an After-Hours Licensed Vocational Nurse (LVN) that will work under the supervision of a Registered Nurse to provide skilled patient care services as outlined in the written plan of care.

Every Saturday & Sunday; 8:30am - 5:00pm

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Assist a registered nurse in providing direct care
• Initiate communication with physicians and managers, IPG members, HHA’s and other health care providers as needed to facilitate optimal care
• Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Manager. Note: The LVN receives direction from the RN on-site and may not alter the patient/family plan of care
• Attend patient conference and participate in discussion of issues to facilitate optimal care

Education/Experience

• Completion of a Licensed Vocational Nursing Education Program from an accredited school
• Minimum of one year of LVN experience preferably in an acute care setting. Experience in a home health/ hospice setting preferred but not required

Certificates, Licenses, Registrations

• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners

Pay Range

$37.65 - $47.00 hourly
