Qualifications

• Bachelor’s degree in a health-related field preferred.
• 1+ year of related health care experience preferred.
• Experience with home health, hospice, or palliative care preferred.
• Knowledge of electronic medical records preferred.

Responsibilities

• Organize and track patient transition records and confirm start of care.
• Identify post-acute care needs and collaborate with Care Transition Coordinator.
• Coordinate with intake teams and arrange outside agency placement if needed.
• Prepare communications, memos, charts, and reports with attention to detail.
• Provide excellent service to patients, families, and referral sources.
• Utilize MS Word, Excel, Access, and PowerPoint effectively.
