Job summary

Ascension at Home together with Compassus is seeking a Weekend Patient Transition Coordinator to support care transitions in Pensacola, FL. The role is remote with local residency required, Saturdays and Sundays 8am–6pm, and may require hospital assistance for coverage.

You will organize referrals to hospice, home health, home infusion, and palliative services, aligning with Care Transition Coordinators to ensure timely services and seamless patient experiences.

Qualifications

• Bachelor’s degree in a health-related field preferred; equivalent experience will be considered.
• Minimum of one year experience in a related health care setting preferred.
• One year experience in home health, hospice or palliative care preferred; EMR experience preferred.

Responsibilities

• Organizes, tracks, and builds a complete medical record for patient transitions to home health providers.
• Confirms start of care (SOC) for each patient referred.
• Identifies post-acute care needs and collaborates with Care Transition Coordinator.
• Completes transition of care including submission of referrals and outside agency placement when needed.
• Maintains confidentiality and delivers exceptional customer service to patients and families.
