Compassus in Austin is seeking a Weekend Patient Transition Coordinator for a full-time, in-person position. The role requires weekend coverage on Saturdays and Sundays, with two possible 5x8 or 4x10 schedules, and local residency.
You will organize medical records, coordinate post-acute referrals to home health, hospice, and palliative services, collaborate with Care Transition Coordinators, and support timely start of care while upholding Compassus values.

This is a full-time weekend role. Schedule must include every Saturday and Sunday coverage, but may be either 5 days/8 hour (Thursday - Monday, 8am - 5pm) or 4 days/10 hour (Friday - Monday, 8am - 7pm) schedule. This is NOT a remote role, you MUST reside locally.
The Weekend Patient Transition Coordinator - Full time supports seamless care transitions by coordinating referrals to hospice, home health, home infusion, and palliative services. This role works closely with Care Transition Coordinators and Home Health Care Transition Coordinators to ensure patients receive timely and appropriate services. The Coordinator promotes Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, upholds the Code of Ethical Conduct, and fosters positive working relationships across the organization and with external partners. Training will need to be completed in person in Austin.
The demands of this role necessate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
