Job summary

Trilogy Health Services, LLC seeks a dedicated Licensed Practical Nurse (LPN) to provide direct patient care and supervise nursing assistants on the unit. You will observe residents daily, respond to changes in condition, and coordinate with charge nurses and physicians as needed.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

• Visit residents on your assigned unit daily to observe and evaluate each resident’s status and notify appropriate personnel of changes.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the LPN scope of practice.
• Assist in monitoring medications, supplies and equipment to ensure adequate availability.
• Perform administrative duties such as completing medical forms, reports and charting as needed.
• Report occupational exposures per facility policies and procedures.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on your assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing (DON) a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Weekend Package Program

• Works a minimum of 2*(8hr) or (12hr) shifts every weekend
• The Weekend Package incentive is an additional $5.00/hour working between the hours of 6pm on Friday to 7am on Monday.
• Additional shift differentials apply when working evening or night shifts on the weekends.
