Job summary

Trilogy Health Services, LLC is seeking an LPN to provide direct clinical care on a designated unit in Boonville, IN. The licensed nurse will supervise nursing assistants and observe residents daily, reporting changes to the charge nurse, physician, or Director of Nursing.

The role includes handling medication monitoring, charting, and administrative duties while maintaining patient safety and quality care. Weekend shifts have extra incentives, and a full benefits package is offered.

Qualifications

• Must hold a current, valid LPN license in the state.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on your unit daily to assess and report changes to the charge nurse, physician, or DON.

• Monitor medications and supplies to ensure adequate stock for residents.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on your assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing (DON) a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Weekend Package Program Details

• Works a minimum of 2*(8hr) or (12hr) shifts every weekend
• The Weekend Package incentive is an additional $5.00/hour working between the hours of 6pm on Friday to 7am on Monday.
• Additional shift differentials apply when working evening or night shifts on the weekends

Qualifications
