Job summary

Lake Sunapee Region VNA & Hospice is seeking a Weekend Overnight On-Call Registered Nurse to provide triage support and after-hours visits for home health and hospice patients in New Hampshire. The role offers autonomy, flexible scheduling, and meaningful patient care relationships.

The position requires NH RN licensure, at least 2 years of healthcare experience, and preferred home health or triage experience. On-call shifts are 4:30pm–8:00am Fri–Sun with a 30-hour guaranteed schedule.

Qualifications

• Current NH Registered Nurse (RN) license.
• Minimum 2 years of health care experience required.
• Home Care experience is preferred but not required; we will train the right candidate.
• Telephone triage experience preferred.

Responsibilities

• Triage all agency patient home health and hospice related phone calls.
• Complete patient communication documentation using the EMR system.
• Collaborate and communicate with team members and program managers.
• Provide home health and hospice visits in patients' homes and facilities.
• Travel throughout the entire service area.
• Other duties as assigned.

Hours:

Salary:

Guaranteed 30 hours of pay

The Weekend Overnight On-Call Registered Nurse (RN) provides triage support and PRN after hours visits to home health and hospice patients. Initial calls handled by on-call clinical triage company prior to escalating to the Weekend Overnight On-Call Registered Nurse (RN). • Ability to triage all agency patient (home health and hospice) related phone calls
