Job summary

Lake Sunapee Region VNA & Hospice is seeking a Registered Nurse for weekend overnight on-call in the New Hampshire service area. You will triage patient calls, document communications in the EMR, and collaborate with a multi-disciplinary team to plan and deliver home health and hospice visits.

Responsibilities include providing in-home and facility visits across the service area, traveling to patients as needed, and maintaining high standards of patient-centered care.

Qualifications

• Current NH Registered Nurse (RN) license.
• Minimum 2 years of health care experience.
• Home Care experience preferred but not required; training provided.

Responsibilities

• Triage all agency patient (home health and hospice) related phone calls.
• Documentation of patient communications using EMR.
• Collaborate and communicate with team members and program managers.
• Provide home health and hospice visits in patients' homes and facilities.
• Travel throughout the service area.
• Other duties as assigned.

Hours

Regular Full-time, Overnight Weekend Friday, Saturday, and Sunday

Shifts

On-Call after hours 4:30pm to 8:00am Friday, Saturday, and Sunday

Salary

Guaranteed 30 hours of pay

Your Responsibilities for the Weekend Overnight On-Call Registered Nurse (RN)

• Ability to triage all agency patient (home health and hospice) related phone calls
• Complete all patient communication related documentation utilizing patient EMR
• Collaborate and communicate effectively with other team members/program managers
• Provide home health and hospice visits in patient homes and facilities
• Travel throughout entire service areaOther duties as assigned

Qualifications

• Current NH Registered Nurse (RN) license
• Minimum 2 years of health care experience required
• Home Care experience is preferred but not required; we will train the right candidate
• Telephone triage experience preferred
