Responsibilities

• Assist individuals with daily living activities (personal care, dressing, grooming, meal prep).
• Provide support with community integration activities (transportation, shopping, leisure).
• Facilitate communication and social interaction, promoting positive relationships.
• Assist with medication administration as directed by medical professionals.
• Implement behavior support plans and provide crisis intervention when needed.
• Maintain accurate documentation of individual support, including incident reports and daily logs.
• Collaborate with team members, case managers, therapists, and family for comprehensive support.
• Participate in ongoing training and professional development.
