Job summary

Sigma Health Rehab is actively seeking a aper diem Occupational Therapy Assistant (OTA/COTA) to work in a Skilled Nursing Facility in Cooperstown, NY. The role involves supporting therapy plans, assisting residents with daily activities, and documenting progress for the supervising OT.

Weekend availability and comfortable collaboration with the therapy team are valued. Current NY OTA license and graduation from an accredited OTA program are required.

Qualifications

• Graduate of an accredited OTA program with NY licensure.
• Ability to select therapy activities per plan of care.
• Experience in or willingness to work in a skilled nursing setting.

Responsibilities

• Select therapy activities following the supervising OT plan of care.
• Assist residents with daily activities and daily living tasks.
• Monitor and document residents’ performance in therapy activities.
• Teach residents how to use equipment to aid daily function.
• Collaborate with OT to establish a rehabilitation plan.
• Maintain cooperative relationships within the healthcare team.
