Responsibilities

• Performs daily tasks of vital signs, blood sugar testing, I&O, EKGs, and assists with activities of daily living under the nurse's direction.
• Provides ongoing monitoring and interaction with residents.
• Records and submits behavioral observations according to policy and standards.
• Assists residents with personal care, activities, safety and daily routine.
• Provides training on basic living skills aligned with rehabilitation goals.
• Demonstrates crisis de-escalation and collaborates with the treatment team.
• Follows AltaPointe policies and participates in required trainings.
• Maintains legible documentation of progress and services provided.
• Supports privacy and confidentiality and maintains a respectful environment.
