Job summary

Angels Care Home Health in Des Moines, IA is seeking a Registered Nurse (RN) for weekend on-call coverage. You will assess patients at home, coordinate with the interdisciplinary team, and ensure the plan of care is implemented and documented.

The ideal candidate has an accredited nursing degree, holds an active RN license, CPR certification, and reliable transportation. Experience preferred, and you will supervise home health aides and LVNs/LPNs as needed.

Qualifications

Graduate of an accredited diploma, associate or baccalaureate nursing program.
Current state RN license and valid driver's license.
CPR certification is required; Hepatitis consent/declination.
Reliable transportation to and from patient homes.

Responsibilities

Cover weekend on-call schedule.
Case manage and provide clinical directions to the interdisciplinary team and family.
Implement, develop, and document the plan of care.
Provide care with infection control measures to protect staff and patients.
Assess home health patients for physical, psychosocial, and environmental needs.
Ensure continuity of quality patient care with proper documentation.
Monitor cases for third-party payer requirements.
Perform on-call duties for patients and families.
Supervise home health aides and LVN/LPN when needed.
Demonstrate commitment, professional growth, and competency.
