Responsibilities

• Provides comprehensive patient care including recording medical history, physical assessments, and developing therapeutic care plans.
• Coordinates with patient, family, community health services and interdisciplinary team; involves physician in plan of care.
• Monitors and ensures continuity of care with periodic physician evaluations as per agreement.
• Prepares written prescription orders and administers medications; performs wound care and sutures as needed.
• Educates patients and families on health, medications, and preventive care; refers to community resources.
