Job summary

Responsibilities

• Provides appropriate patient care, including medical history, physical assessments, therapeutic care plans, and documentation.
• Communicates and coordinates patient care with patients, families, and interdisciplinary team; involves supervising physician in decisions.
• Provides monitoring and continuity of care with periodic physician evaluation as defined in the agreement.
• Prepares prescription orders, administers medications/injections, stitches wounds, and provides wound care.
• Educates patients and families on health, medications, treatment instructions, and community resources.
