Job summary

Capstone Hospice is seeking an empathetic On-Call RN to join the weekend on-call team in Northwest Georgia, covering Saturday and Sunday 8:00am-8:00pm. The role provides skilled nursing care, symptom management, and crisis intervention for hospice patients in their homes or care facilities.

Ideal candidates are Georgia-licensed RNs with bedside experience and a compassionate, patient-centered approach. Part-time, non-exempt, with overtime eligibility and local commuting requirements to Acworth

Qualifications

Georgia-licensed RN with at least 2 years bedside nursing experience.
Graduate of an accredited school of nursing.
Hospice experience a plus (1 year).
Valid driver's license and reliable transportation.

Responsibilities

Respond to weekend patient calls and assess medical needs.
Provide skilled nursing visits to hospice patients in homes or facilities.
Assess patient condition and identify changes requiring intervention or physician notification.
Educate on symptom management using pharmacological and non-pharmacological measures.
Document visits in the EMR system.
Support bereaved families and caregivers during crisis situations and at the time of death.
Coordinate hand-offs with weekday clinical staff to ensure continuity of care.
