Capstone Hospice is seeking an empathetic and responsive On Call RN to join our dedicated on-call team serving the Northwest Georgia area, including Acworth and Canton. The weekend shift (Saturday & Sunday, 8:00am-8:00pm) offers skilled nursing care, crisis intervention, and education to patients and families facing terminal illness.

This part-time role is non-exempt and paid hourly with overtime eligibility.

Qualifications

  • Graduate from an accredited school of nursing.
  • Currently a Georgia licensed RN.
  • Minimum of 2 years of bedside nursing experience (Med-Surg, ICU or CCU preferred).
  • Hospice experience a plus, minimum of 1 year continuous work experience.
  • Current valid driver's license, auto liability insurance and own reliable transportation.

Responsibilities

  • Respond to weekend patient calls, assess medical needs and determine interventions.
  • Provide skilled nursing visits to hospice patients in homes and facilities as needed.
  • Assess patient condition and identify changes requiring physician notification.
  • Provide education on symptom management and pharmacological/non-pharmacological measures.
  • Assist with medication orders, DME and document orders as needed.
  • Develop and re-evaluate the patient/family care plan to maintain continuity of care.
  • Document visits in the electronic medical record system.
  • Participate in post-admission, death and continuous care visits as assigned.
  • Support bereaved families and caregivers during crisis situations and at time of death.

Capstone Hospice is seeking an empathetic and responsive On Call RN to join the dedicated on-call team. This position provides skilled nursing care, assessment, support, and crisis intervention for hospice patients during the weekend hours. This role is responsible for responding to patient and family needs, coordinating care, managing symptoms and providing education and emotional support to patients facing a terminal illness. The On-Call RN works collaboratively with the IDG team to ensure high-quality, compassionate end-of-life care while promoting patient comfort, dignity, and quality of life.

As the On Call RN you will work every weekend, Saturday & Sunday from 8:00am-8:00pm. Candidates should live within a reasonable commute of Northwest Georgia to include the counties of N. Cobb, Cherokee and Bartow counties (Acworth & Canton). This position is paid hourly, non-exempt, eligible for overtime.

The ideal candidate for this role is one who has a passion for hospice care, is empathetic, warm and compassionate. In addition to being a great listener, must be a strong communicator and have the sense to know when to ask more or less questions and be respectful and professional in their demeanor. This position requires someone that can demonstrate and exercise independent and sound judgment to assist patients with complex and diverse issues effectively and efficiently.

Requirements

Candidate must meet the following requirements to be considered:

  • Graduate from an accredited school of nursing.
  • Currently a Georgia licensed Registered Nurse (RN)
  • Minimum of 2 years of recent bedside nursing experience (Med-Surg, ICU or CCU preferred)

To be successful in this role, candidates should possess the following knowledge, skills and abilities:
  • Strong clinical assessment and critical-thinking skills
  • Knowledge of hospice philosophy, symptom management, and end-of-life care principles.
  • Ability to manage urgent situations calmly and effectively.
  • Excellent communication and interpersonal skills
  • Acceptance and adaptability to different social, racial, cultural and religious modes
  • Ability to work independently with minimal supervision
  • Experience or comfortable with pronouncing patient death
  • Proficiency with electronic health record documentation systems.
Essential responsibilities of the role:
  • Respond to weekend patient calls, assessing medical needs and determining appropriate interventions.
  • Provide skilled nursing visits to hospice patients in homes, assisted living facilities, nursing facilities and other care settings as needed.
  • Assess patient condition and identify changes requiring intervention or physician notification.
  • Providing advice on symptom management using pharmacological and non-pharmacological measures.
  • Answering any questions and providing education regarding patient's condition and placing courtesy calls to follow up on patients post intervention.
  • Expectations of assisting medication orders, DME and faxing documents/orders as needed.
  • Develops and re-evaluates the patient/family care plan to meet their needs and maintain continuity of care.
  • Documents visits in electronic medical record system
  • Participate in post admission, deaths and continuous care visits as assigned.
  • Support bereaved families and caregivers during crisis situations and at the time of death.
  • Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
  • Participates in hand-off communication with weekday clinical staff to ensure continuity of care.
  • Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with the HIPAA regulations.
  • Complies with infection control policies and protocols.
Position Factors:
Physical Requirements:
  • Requires considerable physical effort when in-person visits do occur, including, kneeling, squatting, reaching, twisting, climbing, walking, exposure and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient.
  • Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Mental Requirements:
  • Must be able to work independently, make judgements based on assessments and data available and act accordingly.
  • Must be flexible, innovative, and possess good interpersonal skills.
  • Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
  • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, excrements, adverse environmental conditions and hazardous materials.

This position is a part-time and not eligible for Capstone Hospice Health & Wellness benefits.
