Job summary

Capstone Hospice, LLC is seeking an empathetic and responsive On-Call RN to join the weekend on-call team in Northeast Georgia, including Buford. The role involves skilled nursing, assessment, crisis intervention, symptom management, education, and emotional support for hospice patients during weekend hours.

The On-Call RN works Saturdays and Sundays from 8:00am to 8:00pm, living within a reasonable commute of Forsyth and Hall counties.

Qualifications

• Graduate from an accredited school of nursing.
• Georgia licensed Registered Nurse (RN).
• Hospice experience a plus, minimum 1 year preferred.

Responsibilities

• Respond to weekend patient calls and assess needs.
• Provide skilled nursing visits in homes and facilities as needed.
• Assess condition changes and notify physician when required.
• Educate patients and families on symptom management and care plan.
