Job summary

Karen Ann Quinlan Hospice in Phillipsburg, NJ, seeks a part-time exempt hospice nurse to provide comprehensive patient assessment and care coordination. You will explain and interpret patient rights and the Medicare Hospice Benefit when indicated, and collaborate with the nursing team to ensure high-quality end-of-life care.

The role includes administering medications, documenting care, and teaching patients and families, with weekend on-call coverage from Saturday 8:30a to Monday 8:30a.

Qualifications

• Ongoing physical and psychosocial assessment of Hospice patients.
• Follows plan of care and reports observations to team leader or primary nurse.
• Participates with nurse in diagnoses and changes to care plan.

Responsibilities

• Performs ongoing physical and psychosocial assessment of Hospice patients.
• Follows established plan of care for each patient and reports observations to team leader or primary nurse.
• Collaborates with primary nurse regarding new diagnoses and changes in the plan of care.
• Instructs and supervises hospice aides/LPN staff.
• Assists patient/family in obtaining essential equipment and supplies.
• Observes and records signs of disease progression.
• Communicates with physician, family and health team about patient needs.
• Performs patient/family teaching and documents responses.
• Supports patient/family self-direction and independence.
• Delivers medications per policy and documents administration.
• Provides support during Hospice at-home respite and direct patient care.
• Completes charting and returns files within one workday.
• Documents vitals, interventions, treatments and responses; maintains flow sheets and narratives.
