Qualifications

• Hospice care experience preferred.

Responsibilities

• Perform ongoing physical and psychosocial assessment of Hospice patients.
• Follow established plan of care and report observations to team leader or primary nurse.
• Collaborate with the primary nurse on new nursing diagnoses and care plan changes.
• Instruct and supervise hospice aides/LPN staff.
• Administer medications and document administration in accordance with policy.
• Document vital signs and nursing interventions; maintain charts and flowsheets.
• Coordinate with physicians, family and health team to ensure continuity of care.
• Provide patient/family teaching and support self-direction and independence.
• Maintain infection control practices and professional appearance.
