Qualifications

• Graduate of an accredited RN program and valid RN license.
• 2. CPR certification required.
• High School Diploma or equivalent.
• One year of SNF nursing experience preferred.
• Excellent communication and organizational skills.

Responsibilities

• Provide basic preventative, therapeutic, rehabilitative nursing care.
• Develop individualized care plans with the team.
• Administer medications and IV therapy within RN scope.
• Document in EMR and communicate with care team.
• Assist with patient education and discharge planning.
• Collaborate with interdisciplinary team to ensure care quality.
