Job summary

PureHealth is seeking a Registered Nurse to join a post-acute care team. The role supports individualized patient care plans and collaborates with physicians and the health team to ensure quality outcomes.

The RN will administer medications, monitor patient status, and participate in care plan development. Shifts run 6am-6pm or 6pm-6am, with a focus on safety, education, and continuous improvement.

Qualifications

• Must hold a valid RN license.
• CPR certification.
• Minimum 1 year of nursing experience in a SNF (preferred).

Responsibilities

• Provide nursing care in accordance with the care plan and standards.
• Lead and participate in development of individualized care plans.
• Administer medications and IV therapy within RN scope.
• Communicate care needs and updates to the team and physicians.
• Document care activities accurately in EMR.
• Support infection control policies and patient education.
