Qualifications

• Provide direct patient care on weekends with assessment, planning, implementation and evaluation of individualized care plans.
• Collaborate with the healthcare team to develop and implement patient-centered care plans.
• Participate in interprofessional rounds and communicate with patients, families, and other providers.
• Document patient care and outcomes accurately and thoroughly.
• Meet productivity and quality standards while maintaining patient satisfaction.
