Ivy Creek Wellness & Rehabilitation in Waco, TX is seeking a compassionate Double Weekend LPN/LVN or RN to join our patient-centered care team. This role supports 24/7 care on a weekend schedule and provides meaningful opportunities to improve the lives of our patients and their families.

You will work with an interdisciplinary team to monitor conditions, administer medications, and support daily activities.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
• Assist with activities of daily living, such as bathing, dressing, and transferring patients
• Monitor patient conditions and report any changes or concerns to the healthcare team
• Collaborate with nurses, therapists, and other healthcare professionals to develop and implement individualized care plans
• Participate in patient education and discharge planning to ensure a smooth transition to home care
• Maintain accurate and detailed records of patient care and treatment plans
