Aviata Health Group is seeking a Nurse Practitioner with SNF experience to join our family in Florida. You will serve as the SNF’s leading internal medicine provider on Saturdays and Sundays, collaborating with the facility team to reduce hospitalizations and deliver comprehensive care to short- and long-term residents.

The role involves evaluating new admissions, coordinating discharge visits, and managing acute and chronic conditions with prescribed treatments, under established protocols.

Qualifications

• APN with Florida license is required.
• Active license and DEA number required.
• Master’s degree is required.
• Experience in nursing or medical fields preferred.

Responsibilities

Major Responsibilities

• As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
• You will collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends.
• You will work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians
• Evaluation of new-admission visits
• Coordination of discharge visits
• Evaluations for patients exhibiting changes in condition
• As a SNF-ist, you will provide types of care, including but not limited to:
• Advanced Care Planning
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with attending physicians and/or specialists regarding patient care
