Apploi is seeking a Nurse Practitioner (Skilled Nursing) to serve as the weekend lead for internal medicine at a Skilled Nursing Facility in Florida. You will work with an interdisciplinary team, manage care for residents, and drive strategies to reduce hospitalizations on Saturdays and Sundays.

Responsibilities include evaluating new admissions, coordinating discharges, managing changes in condition, and providing comprehensive health assessments along with treatment plans under established

Qualifications

• APN licensure with Florida license is required.
• Active license and DEA number required.
• Experience in nursing or medical fields is preferred.
• Strong clinical background in Infectious disease.

Nurse Practitioner (Skilled Nursing)

Major Responsibilties

• As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services on Saturday and Sunday.

• You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitilizations on weekends.

• You will work with the entire facility's patient population - short and long-term residents with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians

• Evaluation of new-admission visits

• Coordination of discharge visits

• Evaluations for patients exhibiting changes in condition

• As a SNF-ist, you will provide types of care, including but not limited to:

• Advanced Care Planning

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

• Prescribe all necessary medications and treatments

• Perform comprehensive physical examinations of patients

• Develop and implement patient management policies and procedures

• Facilitate referrals to other healthcare professionals and medical facilities

• Communicate with attending physicians and/or specialists regarding patient care

Qualifications
