Job summary

Aviata Health Group seeks an Advanced Practice Nurse (APN) to lead internal medicine services at a Skilled Nursing Facility in Florida on Saturdays and Sundays. You will collaborate with an interdisciplinary team and physicians to reduce hospitalizations during weekends, serving short and long-term residents with support from nursing staff and medical directors.

Qualifications

• Advanced Practice Licensure (APN) with Florida license
• Active license and DEA number
• Previous experience in nursing or other medical fields
• Strong clinical background in Infectious disease
• Ability to build rapport with patients and their loved ones
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
• Master's degree required

Responsibilities

• Provide internal medicine services for SNF on weekends
• Collaborate with interdisciplinary team to reduce hospitalizations

• Evaluate patients with changes in condition
• Provide care including Advanced Care Planning and comprehensive health assessments

• You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitilizations on weekends.
• You will work with the entire facility's patient population - short and long-term residents with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians
• Evaluation of new-admission visits
• Coordination of discharge visits
• Evaluations for patients exhibiting changes in condition

• Communicate with attending physicians and/or specialists regarding patient care
