Job summary

Trumen Physicians + Associates is seeking a Nurse Practitioner to provide weekend post-acute care in East Houston. This independent-contractor role offers autonomy and collaboration with physician-led support, delivering in-person care in SNFs and rehab facilities.

The ideal candidate holds an active Texas NP license with at least 1 year of NP experience (post-acute/geriatric preferred) and can work at least two full weekends monthly (Saturday and Sunday).

Qualifications

• Active Texas Nurse Practitioner license.
• Minimum 1 year of NP experience (post-acute/SNF/geriatric preferred).
• Ability to work independently with clinical judgment.
• Excellent communication and organizational skills.
• Availability for at least 2 full weekends per month (Sat & Sun).

Responsibilities

• Provide in-person weekend care to patients in skilled nursing facilities.
• Manage acute and chronic conditions in collaboration with Trumen-affiliated physicians and facility staff.
• Perform patient assessments, update treatment plans, and review labs and diagnostics within scope.
• Document visits and clinical decisions in the EMR.

Trumen Physicians + Associates is contracting with an experienced and compassionate Nurse Practitioner to provide post-acute care services in the East Houston area on weekends. This flexible independent-contractor opportunity allows you to work any two full weekends (Saturday & Sunday) per month, delivering in-person care to patients in skilled nursing and rehabilitation facilities.
