Job summary

Trumen Physicians + Associates is seeking a Nurse Practitioner for post-acute care in Beaumont, TX. The role offers a flexible schedule and an independent contractor (1099) model with autonomy and tax flexibility.

You will collaborate with affiliated physicians and facility teams to manage chronic and acute conditions, support transitional care, and contribute to quality-improvement initiatives within assigned facilities.

Qualifications

  • Active Nurse Practitioner license in Texas.
  • Minimum of 1 year experience as a Nurse Practitioner, preferably in post-acute/SNF/geriatric care.
  • Ability to work independently while effectively collaborating with the care team.
  • Strong interpersonal skills and a passion for caring for complex patients.
  • Reliable transportation to assigned facilities.

Responsibilities

  • Collaborate with Trumen physicians and facility medical directors to coordinate patient care.
  • Conduct comprehensive clinical assessments and manage treatment plans.
  • Support transitional care planning to help prevent avoidable hospital readmissions.
  • Reconcile medications and provide patient education consistent with scope of practice.
  • Foster positive working relationships with facility leadership, clinical staff, and care coordinators.
  • Collaborate on Trumen-affiliated value-based and quality improvement initiatives.
  • Serve as a liaison between Trumen and designated SNFs.
  • Maintain accurate and timely documentation within the EMR.
  • Maintain an on-site presence consistent with your agreed schedule and facility needs.

About the Role

As a Nurse Practitioner in post-acute care, you will provide in-person care to patients in skilled nursing and rehabilitation facilities. In this role, you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions, support transitional care planning, and contribute to quality-improvement initiatives within your designed facilities.
