Job summary

Avenues Recovery Center is seeking a Nurse Practitioner or Physician's Assistant for weekend coverage at our South Bend, IN location. You will create individualized care plans, monitor medications, and coordinate with medical, clinical, and UR teams to stabilize patients and enable engagement in treatment.

Requires a Master’s degree in a medical field, a valid state license, and DEA certification. Strong communication and collaboration skills with 3–5 years of related experience are preferred.

Qualifications

• Master's degree in medical field and related certifications.
• Valid state medical license.
• Federal DEA certification.
• Acute observational and critical thinking skills.
• Excellent oral and written communication skills.
• Strong collaborative and multitasking skills.
• Preferred: 3–5 years of experience in a related clinical setting.

Responsibilities

• Create individualized medical care plans and monitor/medication management for each client.
• Stabilize patients to enable engagement in treatment.
• Update pharmacy orders, create UDS, med and lab orders, and review UDS and lab results.
• Collaborate with medical, clinical, and UR teams for coordination of care.
• Maintain thorough documentation of all medical interventions and directives.
• Schedule daily and weekly client appointments efficiently.
