Job summary

St. Anthony North Hospital in Westminster, CO is seeking an experienced MSW Social Worker to plan, implement, and coordinate continuing care and community linkages for patients with mental health needs.

This weekend position requires Saturday and Sunday shifts plus every other Monday, with a weekend differential. The role emphasizes interdisciplinary collaboration, thorough assessments, and development of comprehensive care plans to support patient recovery and community integration.

Qualifications

• Master's Degree in Social Work is required.
• BLS from American Heart Association (AED preferred for facilities without AED).
• Valid Driver's License required.

Responsibilities

• Develop and implement patient's plan of care in collaboration with the interdisciplinary team.
• Assess patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, and medical records review.
• Evaluate patient’s response and intervene to optimize outcomes.
• Promote and advocate for patients' health, safety, and rights.
• Demonstrate competency in therapeutic/diagnostic interventions.
• Use quality measures to improve patient outcomes, experiences, and safety.
