Job summary

Claremont Nursing & Rehabilitation Center in Carlisle, PA is seeking a Weekend LPN to join our caring team on a full-time or part-time basis. The role supports subacute and long-term care within a state-of-the-art facility and a warm, homelike environment.

Responsibilities include direct patient care, medications, vital signs, and care plans, with collaboration across healthcare professionals to ensure coordinated care and smooth patient flow on weekends.

Qualifications

• Current LPN licensure in the State of Pennsylvania.
• High school diploma or equivalent required.
• Previous experience in a long-term care or subacute setting preferred.

Responsibilities

• Provide direct patient care as part of a multidisciplinary team, including administering medications, performing vital signs, and conducting treatments.
• Develop and implement care plans tailored to individual patients' needs, taking into account their physical, emotional, and social well-being.
• Collaborate with other healthcare professionals to ensure seamless communication and coordination of patient care.
• Participate in shift bed counts, patient flow, and other administrative tasks as needed.
• Demonstrate a commitment to patient-centered care and a strong understanding of the importance of compassion, empathy, and respect in the delivery of patient care.
