Claremont Nursing & Rehabilitation Center in Carlisle, Pennsylvania is seeking a dedicated Weekend LPN to join our warm, homelike facility. This role supports a multidisciplinary team, providing direct patient care, administering medications, and contributing to customized care plans on weekends.

Responsibilities:

• Provide direct patient care as part of a multidisciplinary team, including administering medications, performing vital signs, and conducting treatments

• Develop and implement care plans tailored to individual patients' needs, taking into account their physical, emotional, and social well-being

• Collaborate with other healthcare professionals to ensure seamless communication and coordination of patient care

• Participate in shift bed counts, patient flow, and other administrative tasks as needed

• Demonstrate a commitment to patient-centered care and a strong understanding of the importance of compassion, empathy, and respect in the delivery of patient care

Requirements:

• Current LPN licensure in the State of Pennsylvania

• High school diploma or equivalent required

• Previous experience in a long-term care or subacute setting preferred
