Job summary

Capital City Hospice in Columbus, OH is seeking an experienced Licensed Practical Nurse to join our hospice care team on a weekend scheduler/triage PRN basis. You will implement care plans, educate patients and families, document care, and coordinate with the inter-disciplinary team to deliver holistic end-of-life care.

The role requires an active Ohio LPN/LVN license, CPR certification, and a heart for hospice care.

Qualifications

• Graduate of an accredited LPN or vocational nursing program.
• Active LPN/LVN license in Ohio.
• 3+ years of nursing experience preferred (Hospice, Med/Surg, or Community Health a plus).
• CPR Certification.
• Strong communication, documentation, and organizational skills.
• Valid driver’s license and insured vehicle.
• A heart for hospice and end-of-life care.

Responsibilities

• Implement care plans based on nursing assessment and patient needs.
• Educate patients and families on diagnoses, treatments, and self-care.
• Document all care and updates accurately and on time.
• Coordinate with interdisciplinary team to provide whole-person care.
• Monitor pain/symptom management and adjust care accordingly.
• Manage incoming calls for medication refills, equipment orders, and patient falls.
• Document incoming calls and interventions provided.
• Managing CC staff schedules and assisting with daily staffing for CC cases.
• Assisting with triage as needed.
