Job summary

Trilogy Health Services in Kalida, OH, is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants on the unit. The role requires daily resident assessments and timely communication with the care team.

The position includes responsibilities for medication monitoring, charting, and clinical duties within the LPN scope, with weekend shifts and incentives as outlined in the Weekend Package program.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate status and notify changes to the charge nurse/physician/DON.
• Identify resident problems and emergency situations and initiate lifesaving measures in absence of a physician.
• Provide holistic care for residents within the LPN scope.
• Monitor medications, supplies, and equipment to ensure adequate availability for residents.
• Complete medical forms, reports, evaluations, and charting as required.
• Report occupational exposures to blood, body fluids, and hazardous materials per policy.
