Job summary

Qualifications

• Current LPN licensure in Ohio.
• Minimum 1 year of experience as an LPN (new graduates welcome).
• Ability to work a full-time schedule, including days, evenings, and weekends.
• Strong clinical skills including medication administration, vital sign assessment, and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment with multiple priorities.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and conducting treatments as ordered.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Monitor and report patient progress to the healthcare team, family members, and other stakeholders.
• Maintain accurate and detailed records of patient care and medication administration.
• Participate in quality improvement initiatives to enhance patient outcomes and staff development.
• Communicate effectively with patients, families, and colleagues to ensure seamless care transitions.
