Job summary

CareCore Health LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our nursing team as a Floor Nurse in Ohio. You will provide direct nursing care to residents and support daily unit operations, ensuring the highest standards of care.

The role includes MED administration, wound care, and collaboration with the care team. A compassionate, patient-centered approach is essential, with full-time hours and every-other-weekend rotation.

Qualifications

• LPN license in Ohio is required.
• Current CPR certification.
• Ability to work collaboratively in a healthcare team.
• Compassionate, patient-centered approach to care.
• Long-term care or similar setting experience is a plus but not required.

Responsibilities

• Provide nursing care to residents per individualized care plans.
• Administer medications, treatments, and wound care as prescribed.
• Assist with admissions, discharges, and transfers of residents.
• Document resident care, medications, and treatments accurately.
• Communicate with residents, families, and healthcare team members.
• Assist STNAs and other team members to ensure high-quality resident care.

Position Overview:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our nursing team. As a Floor Nurse, you will provide direct nursing care to our residents and assist in the daily operations of the unit. This role is vital in ensuring the highest standards of care are maintained for our residents.

Schedule:

Full- Time 6:00am-6:00pm with an every other weekend rotation

Responsibilities:

• Provide nursing care to residents in accordance with their individualized care plans
• Administer medications, treatments, and wound care as prescribed
• Assist with admissions, discharges, and transfers of residents
• Ensure accurate documentation of resident care, medications, and treatments
• Communicate effectively with residents, families, and healthcare team members
• Assist STNAs and other team members as needed to ensure high-quality resident care
• Comply with state and federal regulations, facility policies, and procedures

Qualifications:

• Valid LPN license in the state of Ohio
• Current CPR certification
• Ability to work collaboratively as part of a healthcare team
• Compassionate and patient-centered approach to care
• Previous experience in long-term care or a similar setting is a plus but not required
