Innovive Health is seeking a Licensed Practical Nurse (LPN) for Home Health on weekends part-time. You will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse, delivering patient-centered care and supporting independence.

This role offers flexible weekend schedules, one-on-one patient care, and an opportunity to collaborate with an interdisciplinary team while avoiding overnight shifts.

Competenze

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.

Mansioni

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and other healthcare team members.
• Educate patients and families on disease management, medications, and treatment plans.
