Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) for in-home care on a PRN weekend schedule. You will provide direct patient care, monitor vitals and progress, and collaborate with a care team to ensure comfort and safety for clients.

Educational requirements include a practical nursing program and a current LPN/LVN license in the state(s) served. This role offers competitive pay and flexible scheduling to fit your life.

Qualifications

• Graduate of a practical nursing program (LPN/LVN)
• Current unrestricted license to practice as LPN/LVN in the relevant state
• Reliable transportation to care sites/work locations
• Practical trach/ventilator experience preferred, not required

Responsibilities

• Provide in-home direct patient care according to the plan of care while monitoring progress
• Assess and document patient conditions and report changes (vital signs, fluids, glucose)
• Administer medication, insulin, and IV/fluids with thorough documentation
• Inspect and care for wounds, change dressings and assist with personal hygiene
• Assess for injuries, infections, bedsores, and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure patient outcomes
