Job summary

Innovive Health seeks a Licensed Practical Nurse (LPN) to join our Home Health team in Massachusetts. You will provide high-quality nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team, helping patients maintain independence and improve quality of life.

This full-time role offers a predictable Monday–Friday schedule with no overnights, weekends, or holidays.

Qualifications

Graduate of an accredited Practical Nursing program.
Current, unrestricted Massachusetts LPN license.
NCLEX-PN completed.
Current CPR certification.
Strong clinical assessment, communication, and documentation skills.
Valid driver's license and reliable transportation.
Ability to meet health screening and clearance requirements.
Professional references demonstrating competence.

Responsibilities

Provide skilled nursing care per care plans.
Administer medications and treatments safely.
Monitor patient conditions and report to supervising RN.
Collaborate with physicians, case managers, caregivers, and healthcare team members.
Educate patients and families on disease management and treatment plans.
Maintain accurate clinical documentation.
Support patients with dignity, compassion, and respect.
Participate in care conferences, team meetings, and ongoing training.
