A healthcare service provider in Pennsylvania is seeking an LPN Caregiver to provide personal care assistance to elderly and convalescing patients, maintaining a clean and safe environment. Responsibilities include monitoring clients' conditions, executing care plans, and assisting with personal hygiene and meals. The ideal candidate should possess strong organizational skills, a compassionate attitude, and a current PA LPN license. Flexible scheduling options and both full-time and part-time positions are available.

Qualifications

• Ability to read, write, and communicate effectively.
• One year of home health experience preferred.
• Familiarity with feeding tubes, assisted breathing devices, and Hoyer Lift.

Responsabilités

• Providing frail or immobile clients with assistance and comfort.
• Executing the patient care plan and communicating with RN Case Manager.
• Monitoring the conditions of clients and reporting changes to designated parties.

Formation

Graduate of an accredited LPN program

Current LPN license (PA)
