Job summary

A leading home health care provider in Pennsylvania is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care under supervision. The role involves assisting with patient care plans and providing education on self-care techniques. Candidates must have a valid LPN license, at least one year of relevant experience, and CPR certification. Qualifications

Responsibilities

• Assist the Registered Nurse in implementing and carrying out the plan of care.
• Perform specialized procedures under the supervision of the Registered Nurse and according to physician orders.
• Participate in the preparation, implementation, and education of patient care plans.
• Recognize and address the effects of social and economic challenges on patients.
• Educate patients on appropriate self-care techniques.
• Prepare clinical and progress notes (excluding admissions).
• Assist in patient rehabilitation according to the medical care plan.
• Actively participate in nursing organizations and in-service programs.
