Qualifications

• Current LPN license in state is required.
• At least one year of LPN experience in a long-term care setting preferred.

Responsibilities

• Provide bedside care to patients including personal hygiene maintenance.
• Record patients' vital signs (blood pressure, pulse, respiratory rate).
• Collect patient health information and document in medical records.
• Relay information between patients, RNs, and physicians.
• Administer oral medications and injections as directed.
• Assist patients with personal hygiene and daily activities.
• Monitor for changes in patients and report promptly.
• Maintain confidentiality of patient records and proper charting.

LPN Responsibilities:

• Provide bedside care to patients including personal hygiene maintenance
• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)
• Collect patient’s health information
• Relay information between patients, registered nurses, and physicians
• Give oral medications and injections to patients
• Assist patients with personal hygiene
• Monitor for changes in patients
• Proper charting, documentation, and maintenance of patient’s medical records
• Asks questions when in doubt for clarification by the Registered Nurse.
• Acts as a role model for ancillary personnel.
• Maintains the confidentiality of the patient’s clinical records.

LPN Requirements:

• Current LPN license in State.
• At least one year of experience as a LPN in a Long Term Care setting preferred.
